Welcome back to The Digital Counter, I am Wei Ling Tan. Last time we looked at a system that works. Today we look at one that did not. A benefits payment system in another country matched income data automatically, decided thousands of people had been overpaid, and sent letters demanding money back. Many of those letters were wrong. There was a public inquiry, and the record is long and detailed, so we are going to read it carefully with a public sector auditor who has reviewed failures like this before. We will look at the data match that seemed right, the human review step that was missing, the appeals, and what the inquiry recommended. I will end with the lessons I would take into any service that moves money. You are listening on Witspeak Podcasts.
